Application and Submission –YOU MUST SAVE THE APPLICATION TO YOUR COMPUTER AND EMAIL OR FAX–

- Save the application form to your computer. Once you have saved the application, you can then complete the application electronically along with necessary signatures. Once the application is completed, you can then email it to the email address below, or you can print the application and scan it back into your computer to then be emailed, or you can fax the application along with your proper identification for employment. PLEASE NOTE: This file works best in Internet Explorer.
- Scan or Fax proper identification for employment i.e. 2 forms of identification such as a Photo ID (Government Issued) and Social Security Card or Birth Certificate unless you can supply a List A, i.e. Passport, or Permanent Resident Card etc…
- Submit your proper forms of Identification and your completed application/medical coverage forms via email:firstname.lastname@example.org or fax: 910-754-5433 In order to process your application, you MUST complete MEC/Fixed Indemnity Enrollment forms. Upon hire, you will have 30days to make any changes.
